If your vehicle registration is lost or destroyed…
Submit the Affidavit of Loss section of the Affidavit of Loss/Release of Interest form.
- All registered owners must have their signature notarized on the form.
- Include check/money order for the fee payable to: Department of Licensing. Fees vary, contact a vehicle licensing office to get the correct amount for your situation.
- Mail or bring the form to any vehicle office.
